Who does the headphone mix, you or the musicians?

I'm re-evaluating my headphone mixing system/technique so I'm curious who controls your headphone mixes, and where?

a) by engineer in the control room
b) by musicians in the live room

I also wanted to split the answers betwee "part time" studio and "commercial" studio. Let's define "commercial" for this exercise as receiving a majority of your income from the studio.

We used to have little mixer stations in the live room for the clients, but finally found that it was more trouble than it was worth.
You'd be suprised just how many musicians have no idea how to work a simple mixer.
Sometimes, we'd spend more time in the live room setting up a mix for them than we would be recording anything.
A lot of them would just turn everything up to 11 and say, "I want my guitar louder, but it's already all the way up on the thingy."

We're back to aux sends feeding power amps that feed Rolls PM52's.
