I've used a 416 for vocals on occasion when it was my best option in a loud/live situation. Nobody said, "sounds good except for the vocal." Maybe that only means it was all equally bad. :-)

I apologise for mixing up ADR and voice work. Honest mistake after a long day! I have done not only animation though, but also voiceover and audiobook work. Same experience there.

Sounds like a very annoying way to spend one's day, but I guess it's all in a day's work! I honestly hear the dialogue even in big budget films, and I'm often shocked at how poor the sound quality is. But, that's what can physically be achieved in real-world video shoots, I guess, unless the whole film is completely dubbed.

The way the ADR and dialogue are recorded is, again, only tangentially related to the matter at hand. Shotgun mics are a poor choice for vocal recording in small untreated rooms.
