OMF/AAF - tips for creating interchange files

After helping one of my clients solve a problem creating an OMF from Premier, he suggested I create a tut on my blog. In the past I've found many video editors who struggle to create good OMF files. I put together some basic info and would like your help.

Lots of you guys/gals have more experience with this. Please check out the post and feel free to offer up corrections, suggestions, additions, etc.

How about a word or two in there about keeping your tracks consistent. Dialog on the dialog track, VO on the VO track, PFX somewhere other than between two cuts of music...

I sometimes think video editors are afraid each additional audio track will come out of their salary, and they have to jam each audio clip on the topmost available space in their timeline.
